• Have your periods started to be delayed?
• Are we starting to gain some weight? Or has the belly area started to look like a light bagel? Are we getting thicker?
• Where did these pimples come from?
• Where did this hair growth come from? Hairs started growing on my chin, between my breasts, even around my navel……
….if you say; it means it’s time to see a gynecologist without delay…..
This condition, which we call polycystic ovary disease or syndrome, starts to show itself with signs of delay in menstruation, weight gain – obesity (increase in waist-hip ratio, increase in favor of waist), male pattern hair growth, and may even be a cause of infertility in the future.
As a word, polycystic ovary means ovary containing many cysts. On ultrasonography , 2-3 mm diameter , approximately 10-15 pearly cysts are observed . The reasons for these are the deterioration of the hormone balance in women (FSH/LH ratio in the direction of LH increase), the increase in male hormones (testosterone and its derivatives).
These hormonal disorders have negative effects on women’s health in the long term. Such as increased cholesterol, cardiovascular diseases, insulin resistance, diabetes mellitus, infertility, increased risk of endometrial (uterine) cancer, obesity.
It is also thought that polycystic ovarian syndrome may be genetic, but it is not known at what time it started in the woman’s life. Studies have shown that this developmental process can begin in the womb, and other studies have shown that it begins in adolescence.
Due to hormonal and functional disorders, the formation of small cysts in the ovaries, their growth and remaining 8-10 mm in diameter, and if not treated, a vicious cycle may begin. As the ovaries turn into honeycombs, the ovulation function will begin to disappear and the menstruation will begin to be delayed. Since there is no ovulation, inability to conceive can be an important problem in itself. Irregularity in metabolic balance will cause insulin resistance, increase in fasting blood sugar, susceptibility to diabetes, weight gain and obesity, imbalances in male hormones such as increase in testosterone and its derivatives in hormones will cause acne (acne), hair growth, and these hairs will become male type.
How can this vicious circle be broken?
I have polycystic ovaries, will I not be able to conceive?
Will I get uterine cancer?
Will these pimples go away?
I’m tired of these hairs…
The first thing to do is make minor changes in our lifestyle. What could these be?
We can lose weight, lose weight, not gain too much, and break the insulin resistance and the tendency to obesity by weight control, losing weight, eating a balanced diet.
Medical treatments and support include reducing the circulating androgen load (male hormones), supportive treatments in women who want pregnancy (helping ovulation), protecting the endometrium (intrauterine layer), preventing uterine cancer, diseases that may occur after insulin resistance (glucose intolerance). , diabetes) will prevent.
Medical treatments are performed for acne (pimples) and hair growth. Shaving is not recommended for hair removal, waxing can be done. However, first of all, medical treatment and suppression of hair growth should be provided. In conditions where epilation methods are tried without treatment (without hormonal balance), the hairs will continue to grow again and will begin to acquire a terminal masculine hair structure. If women with hair growth complaints are overweight, they should definitely lose weight.
With the drugs we call insulin sensitizers (metformin), insulin resistance is tried to be broken, it contributes to weight loss, and they are used for treatment purposes, considering that they also help in conceiving. In 3-6 months of metformin use, improvement in menstruation and reductions in hair growth are noted with longer use. It increases the chance of conception and has been shown to reduce the risk of miscarriage in pregnancies.
In women who do not want to get pregnant, birth control pills both ensure the regularity of menstruation and reduce the risk of cancer by keeping the thickness of the endometrium (the inner layer of the uterus) under control.
When polycystic ovary syndrome is detected at the initial stage, it will be easier to achieve success in treatment with lifestyle changes, diet, sports-exercise-weight loss…
