In my opinion, rhinoplasty, or rhinoplasty, is the most sophisticated operation of plastic surgery. The cartilage and bone structure that gives its shape under the skin is reshaped by changing the angles with the help of thin sutures and putting the excess in the missing places. The most important issue that the patient should consider; It should be that a nose was not made from scratch, but changes were made to the old nose skeleton. In other words, requests and expectations should not be evaluated as “as raised or small as another nose”, but as “more raised or smaller” compared to the old nose. Nasal surgeries are performed with two techniques, open and closed technique. The open technique is in the form of making an incision from the tip of the nose and making the skeleton fully visible, and the trend all over the world is in favor of this surgery. I also use this technique at a rate that I can say 80% in my surgeries. The other technique is the closed method, where the incision is made from the inside of the nose and the skeleton is not fully visible. Both methods have their own advantages. It is not important which method is better, but which method suits you better. In nose surgeries, “unbuffered surgery” is mentioned, in my opinion, this is a situation related to the patient’s needs, not the surgeon’s style. If there is a curvature (septum deviation) in the inner-middle wall of the nose, which we call the septum, and if the septum needs intervention, I use tampons, otherwise I do not use it. In patients I use tampons, I withdraw the tampon after 3 days. The rumors that tampon removal is painful are not true, in fact, tampon removal is a completely painless procedure that can be done with some care and attention. I perform my rhinoplasty surgeries in the operating room and under general anesthesia. I usually hospitalize my patients for one night and send them the next day. After 3 days, if I put it in, I pull the tampon and 1 week later I take the splint on the back of the nose. If my patients have bruises and swelling (swelling is certain for 2 to 5 days, but bruising is not always seen), all swelling and bruises on the face go away within 7-10 days. The swelling in the nose is greatly reduced in 1 month, but it takes 6-9 months for the patient’s nose to take its final shape. Warnings and precautions: At least one week before the operation, blood thinners such as aspirin, coumadin, food supplements such as vitamin E, Ginko biloba should be discontinued. Blood pressure should be regulated. After the operation, the patient should lie on his back with the head slightly elevated. It is necessary to avoid activities that will tire the eyes for a while. When the first splint is opened, it is normal for the patient to encounter a nose that is beyond his expectation, even a very beautiful nose is a danger sign. It should be taken into consideration that the swelling of the nose will decrease.
