Gynecology, Obstetrics and IVF Specialist Op. Dr. Seval Taşdemir told what should be known about unexplained infertility.
More than half of the couples who apply to the doctor with the desire to have a child have unexplained infertility. Failure to achieve pregnancy at the end of an unprotected year in cases where the ovaries of the expectant mother are working regularly, the tubes are open, the uterus is healthy and the sperm values of the father-to-be are normal; considered unexplained infertility. A significant number of couples with unexplained infertility, which we cannot diagnose with current technologies, are able to conceive spontaneously without any help. For these couples, it is not possible to explain the previous failures and the pregnancy achieved over time.
20 out of 100 couples who do not have health problems cannot get pregnant for unknown reasons. If there is a problem in the reproductive system, pregnancy is prevented. The pregnancy chance of problem-free couples, which is around 15-20 percent every month, decreases to 3-5 percent in unexplained infertility groups. In other words, the possibility of spontaneous pregnancy still continues in these couples. In unexplained infertility; It is considered to be a temporary or permanent, simple or complex problem in the reproductive system. Sometimes there can be multiple problems with different steps and mechanisms.
Infertility and age are important
If the duration of infertility is less than four years and the age of the expectant mother is below 35, vaccination should be the treatment of choice. If the mother’s age is over 35, vaccination can still be done, but IVF treatment should be started without increasing the number of vaccinations. IVF is always an available treatment for couples. However, easy treatments should be tried first. Couples who cannot be successful with these methods should then be treated with IVF.
In the vaccination treatment, it is ensured that the sperm come together with the egg at the right time. Egg-enhancing drugs are used to develop one or two eggs by pills or injections. When the egg reaches a certain diameter, spawning is carried out with the help of cracking needles. This period is the time when the chance of pregnancy is highest. Ovulation occurs approximately 36 hours after the fracturing injection. In an hour close to this time, sperms are taken and preparation and washing process is carried out in the laboratory. Then, the fastest and healthiest ones are collected and injected into the uterus with the help of a soft catheter.
In IVF treatment; More drugs are used than vaccination and follow-ups are more frequent. When the diameters of the follicles reach a certain size, eggs are retrieved by using vaginal ultrasonography with a fine needle 36 hours after the cracking needle, as in the vaccination treatment. In the laboratory environment, each egg is cleaned from the surrounding cells and evaluated under a microscope, and a sperm is injected into each mature egg with the microinjection method. Embryos developing after fertilization are followed in incubators and selected embryos are transferred to the mother’s womb.
Stress is the most common factor in unexplained infertility. Stress, which disrupts the harmonious functioning of the mechanism consisting of emotional, mental and physical integrity, also negatively affects the possibility of conception. Women who expect menstrual day calculation, intercourse timing, ovulation tracking and menstrual delay are faced with stress much more. As the chance of pregnancy decreases for women who do not receive enough support from their husbands, the stress increases exponentially. However, if couples stay away from stress and support each other, the probability of having a baby increases.
