Breast (breast) augmentation surgery, in its most common form, is a surgical technique that allows to increase or restore the size of the breast by placing an implant (silicone) on the breast. Every year, hundreds of thousands of women in the world have this surgery for different reasons.

The most common causes can be listed as follows:

Increasing the size of the natural breast

Correcting the asymmetry in the breasts (different breast sizes)

To restore breast volume lost due to weight loss, pregnancy or breastfeeding

Having a better body shape and proportion and feeling better

This surgery is called ‘breast augmentation surgery’. In breast augmentation surgery, silicone is placed in the appropriate area with a small skin incision. The recovery period usually requires rest ranging from 24 hours to 48 hours.

In your first pre-operative examination, Assoc. You will meet Haldun Kamburoğlu. In this examination; Your general health status, the diameter, shape and size of your own breasts will be examined and the most suitable method will be explained to you. In addition, special conditions (breast asymmetry, rib cage deformities, breast sagging, etc.) and their treatments, safety issues and the postoperative period will also be explained to you. With the professional and expert guidance of your doctor, you will determine together the breast implant size and from which area (under the breast, under the armpit) the implant will be placed.

It is recommended that you do research on breast augmentation surgery before you go to your first appointment with your doctor. Bra sizes consist of a number and a letter. The number determines your under-breast circumference, and the letter determines your breast size. You can get an idea of the size by trying on various bras. However, an important issue that you should pay attention to is that the sizes corresponding to these letters may differ among the bra manufacturer countries. Do not get hung up on prosthetic volumes (cc). A prosthesis volume that may be insufficient for you may be quite large for another patient. The width of your rib cage and your height are two of the most important parameters that allow us to decide. In summary, we can say that if you are having augmentation surgery, placing the most suitable natural prosthesis that your rib cage can take will give the best results. There is a very fine line between big and natural and big and ugly in augmentation surgeries. The most important factor in determining this is the experience and taste of your surgeon. Your surgeon works while you sleep during the operation. It wakes you up and says ‘do you like it?’ It is impossible to ask. Therefore, try to understand deeply the taste of the surgeon you decide to have surgery.

In order to determine the size of the prosthesis to be placed, Associate Professor Haldun Kamburoğlu will perform very detailed measurements during your examination. Then, analog simulation will be made in front of the mirror with silicones of various sizes, and thus the prosthesis suitable for you will be determined. Your body will be scanned in 3D with our Vectra 3D device so that you can see how this prosthesis will look on you as closely as possible to the result of the surgery. Our systems will digitally simulate the state of the silicone we have chosen for you. Yes, in this way, you will have a clear answer to the question ‘what size’ you are most curious about at this point.

Breast implants are produced in two forms as drop (anatomical) and round. The outside of these implants is a silicone sheath, and the inside is a silicone gel. New generation implants are very resistant to impacts and pressure. (There is a video about this on the facebook page of Associate Professor Haldun Kamburoğlu.) Today, drop (anatomical) prosthesis is more preferred in standard breast augmentation surgeries. The reason for this is that a much more natural result can be obtained with a drop prosthesis. In addition, the silicone gel in drop dentures is produced with a much more advanced technology than round dentures. As a result, according to the 10-year results of the manufacturers, complications such as capsular contracture, seroma or deformation of the prosthesis (this also causes sagging under the breast) in drop prostheses are less than in round prostheses. In other words, sagging in drop dentures is less than in round dentures due to superior production technologies, contrary to popular belief. On the other hand, due to the necessity of surgical technique, some patients may also prefer round prosthesis in patients who have breast lift and augmentation at the same time. Associate Professor. A frequently asked question to Haldun Kamburoğlu is the possibility that the drop prostheses can rotate inside. Although this possibility exists as a book, none of his patients have experienced this or a similar problem so far. He prefers Mentor brand prostheses approved by the American Food and Drug Administration (FDA) in his surgeries.

Before Surgery

Before going into surgery, it is important to consider the following points:

In your pre-operative examination, Assoc. Some analyzes necessary for you to receive general anesthesia will be performed 1 week before the operation and the results of these will be evaluated by the anesthesiologists.

Before the operation, you should make sure that you do not have a problem with your health. It is important to be in good physical condition because this reduces the risks during the surgery and provides a faster recovery process. If there is a treatment or treatments you are currently undergoing, you should notify Associate Professor Haldun Kamburoğlu. Our aim; to minimize possible complications, to increase the result of the surgical application and to provide you with the highest patient safety conditions in today’s current conditions.

Aspirin etc. You should not use blood thinners containing ibuprofen and flu medications at least 1 week before and 2 weeks after surgery. In the presence of Coumadin and anti-rheumatic drug use, the treatment protocol is slightly different for blood thinners and will be explained by your doctor.

If you are using birth control pills, you should stop completely for 1 month before and 1 month after surgery. Roacuttan etc. If you are using drugs, it is recommended not to use them for 6 months before and 6 months after surgery.

Substances such as vitamin E, garlic, cherry, ginseng, green tea, herbal teas, flaxseed, resveratrol (red wine extract) can also cause blood thinning. For this reason, it is recommended that you do not take these substances until at least 1 week before the surgery. It is recommended that you do not use these substances for at least 2 weeks after the operation.

Call and inform Assoc.Dr.Haldun Kamburoğlu about all kinds of diseases and all medications you need to use (including all painkillers) within 3 weeks before the operation.

If you smoke, it is preferable to stop smoking 4 weeks before surgery to reduce the risks of general anesthesia.

Take a bath 1 night before the operation.

Bring a button-down pajamas on the way to the hospital. Do not bring overnight.

The use of 1000mg vitamin C once a day for the 2 weeks before and 2 weeks after the surgery will help speed up your wound healing.

If there are hairs around your nipple before the operation, you can cut them with scissors. However, pulling them out with tweezers is inconvenient due to the possibility of folliculitis (inflammation of the hair follicle).

In order to receive general anesthesia, you must be hungry for 8 hours and dehydrated for 4 hours before the operation.

Associate Professor. Haldun Kamburoğlu begins the surgical process with a small incision of 3.5 – 4 cm from the incision under the breast, which can be easily hidden in the inframammary fold. The incision under the breast is brought exactly to the new inframammary fold so that the scar is not visible, so it is difficult to see the scar even when naked. He does not prefer patients who wear the underarm incision with a sling, as it is possible to see the scar when they raise their arms (especially unless the patient requests it). Similarly, nipple incision; It is not preferred due to possible loss of sensation, visible scars and the possibility of infection. In the next step, the implant is placed through the opened incision. The implant is placed in the area with the submuscular method, the supramuscular method (just behind the breast tissue), the submuscular method or the dual plane method (a more advanced version of the submuscular method). Associate Professor. Although Haldun Kamburoğlu prefers the dual plane technique, which is the most up-to-date and successful method, he decides which method to use, taking into account the type of implant, the size you want to enlarge and your specific anatomical needs. The under-breast tissue method is also a suitable choice for individuals with sufficient skin thickness and active sports. On the other hand, the submuscular method, which has been popularized in the last 10 years, is not preferred in clinical studies published in the literature, since no superiority has been demonstrated over the sub-breast tissue method and it also causes prolongation in the healing process.

At the end of the surgery, the nerves between the ribs are anesthetized and a technique that will minimize your post-operative pain is also applied. When the breast augmentation surgery is completed, the incision will be closed with a dissolvable aesthetic suture. There is a small surgical scar at the beginning, but with careful and careful care, it will decrease or even disappear over time. Associate Professor. Haldun Kamburoğlu does not use drains (hose system that transfers possible blood collections in the body) in any primary breast augmentation surgery (if there is no vascular abnormality). This is because the use of drains can increase the likelihood of developing a capsular contracture.

Recovery time after surgery for healthy individuals includes 48 to 72 hours of rest. For your pain control and comfort, it is preferred that you stay in the hospital for one night. This is not medically necessary. If desired, you can be discharged on the same day. Dr. Kamburoğlu will ask you to reduce your daily activities for the next few days. The length of this process will depend on your current health condition and the appearance of your surgical scar and its healing status during the post-operative check-ups. Details are below.

You should not eat or drink anything for a while after the surgery. During your stay in the hospital, the nurses will tell you when you can start drinking water. Start drinking water at the 4th hour of the operation. In the 6th hour, you can gradually switch to solid foods. (You can consume all kinds of food after 8 hours. It is recommended to consume plenty of water for 24 hours after you start drinking water.

Please inform the nurses (or Dr. Kamburoğlu if you have been discharged) if you do not pass rectal gas or stool in the first 24 hours after the operation.

You should walk in the company of a companion and nurse as soon as possible after the surgery (when the nurses measure your blood pressure and say that there is no problem in walking). It is also recommended that you move your legs and feet inside the bed as long as you lie down. The more you walk during your stay in the hospital, the more the risk of clot formation in the leg veins, which is one of the common risks of anesthesia with all surgeries, is prevented. As a safety precaution during your surgery, a special machine is operated on your legs for this purpose.

After the surgery, you will have a bandage around your chest. This bandage will be removed the next day and you will be put on a special bra and chest strap. This bra and chest band will be brought to you by the relevant medical company in the morning and the cost will be collected by the company at the hospital. Do not have any expectations about the shape and size of the bra when the bandage is removed and the bra is put on. Because the prosthesis tries to expand the skin from the inside and the bandage presses from the outside, the image is almost always not as imagined. But don’t worry, here’s a detailed description of how to shape and achieve just the beauty you want.

During the surgery, Associate Professor Haldun Kamburoğlu will apply drugs to your chest muscles and ribs to minimize your pain. In this way, your pain will be less for the first 6 hours after the surgery. Then, in order to alleviate your possible pain, a pain-relieving device that gives intravenous drugs will be with you when you press the button in your hand. The pain of breast augmentation surgery is felt quite acceptable thanks to the medications given in the hospital on the first night and the medications taken orally on the second and third days. From the 3rd day, your pain disappears dramatically in the vast majority of patients.

It is recommended that you stay in the hospital for 1 night after the operation so that you will be under observation. Associate Professor Haldun Kamburoğlu will come to discharge you between 11:00 and 12:00 the day after the surgery. Your discharge time is 12:00 noon. During your discharge, you will be prescribed the medications listed here and some additional medications.

In the first 2 weeks after the surgery, your breasts may look slightly larger than you expected, especially due to the swelling in the upper region.

At the end of the 2nd week, the swelling in your breasts starts to go down. At this stage, you may be afraid that the breasts have shrunk, but do not be afraid, because the volume of the prosthesis inside is fixed and the final size and shape has not yet been reached, as your breast skin has not yet been stretched and shaped by the prosthesis.

In the first month after the operation, if you have a sufficiently flexible skin, your breast is quite shaped. However, if you have a very tight skin and have no breasts before the surgery, you will have to wait until the end of the 3rd month for this shaping.

You can take a bath 2 days after the operation. Your first dressing will be done by Assoc.Dr.Haldun Kamburoğlu at the hospital just before discharge. What you need to do as a dressing is to remove the gauze on your wounds and enter the bathroom 2 days after the operation, before entering the bathroom. (Don’t pull it up or down.) Then, apply Terramycine eye ointment to the wound, which will be used once a day after bathing, and cover it without taping it with a sterile gauze (taping can cause allergies, the gauze will keep the bra in place anyway). Wear the bra worn in the hospital, which you should wear for the first month.

At the end of the 1st month, you can remove the medical bra and get any bra with or without underwire. But you have to wait 3 months to use a supportive bra for an extra low-cut look.

It’s normal for your breasts to be hard at first. You will see that it starts to soften in the 3rd month and becomes normal enough to touch each other in the 6th month. And in the first year, your breasts gain a consistency similar to natural breasts with an extra softening.

It is VERY IMPORTANT that you do not sleep on your side for the first 1 month. You can sleep on your side at the end of the month. You may have back pain from lying on your back during this time. Electric massage devices sold in electronic markets help many patients in this regard.

You can swim 2 weeks after the operation, but you cannot swim. You can swim slowly in the 1st month, and you can swim competitively in the 3rd month. In summary, you cannot do any sports for the first month. After the first month, you can do sports such as running, cardio, swimming very slowly, and after the third month, you can do sports such as push-ups, tennis, fast swimming, fitness, where you need to use chest muscles.

It is recommended to use sunscreen (50 factor and lotion form) on small marks on your breasts (silicone gel 2 times a day, for 2 months) from the 7th day. Even if you’re in a bikini, you should apply sunscreen every time you go out to sunbathe (or under your bra if you’re wearing a see-through outfit). The important point here is to apply sunscreen first. Because it makes a trace-reducing silicone layer, it does not pass sunscreen under it.

Massage application after breast augmentation surgery is not routinely recommended as it can cause double capsule formation and slippage according to the current literature, and at the same time, no preventive effect on the development of capsule contracture has been demonstrated. You can use an aloe vera-based cream or body lotion as a moisturizer.

There is no need to take stitches since dissolvable aesthetic stitches are used in the body. Your check-ups by your doctor will continue at regular intervals until you complete the first year of your surgery. These ranges vary from person to person. Afterwards, it is recommended that you come for a check-up once a year.

Women before the age of 40 who do not have breast augmentation surgery should have breast imaging with ultrasonography once a year, and after the age of 40 with ultrasonography and mammography once a year. It is okay to have ultrasonography (USG) and/or mammography from the 6th month after breast augmentation. If it is difficult to compress your breast with the mammography device, then do not try to have a mammogram. Mammography is a diagnostic test with high sensitivity and relatively low specificity. If any lesion is suspected in this test, the next step test (its specificity is much higher) is MRI. For this reason, we recommend having an MRI once a year for our patients whose breasts are difficult or unwilling to have their breasts compressed. MRI is also a completely safe test that does not contain radiation like mammography. On the other hand, we recommend breast control with MRI every 3 years for all age group patients who have annual follow-up with USG and/or mammography.

Although the risks are minimal, regular post-operative care is very important. As with any surgery, you should use your common sense during the healing process of breast augmentation surgery. In case of any doubt, you can call Dr. Kamburoğlu directly. The phone number of Associate Professor Haldun Kamburoğlu is 0 532 473 35 02. You can reach him by whatsapp or by phone. If he can’t answer when you call, he will definitely get back to you.

There is no harm in planning a pregnancy after breast augmentation surgery. The silicone protz placed in the breast augmentation process does not prevent you from giving milk, it does not pass to your baby with the milk you give. On the other hand, if your new breastfeeding is over, you have to wait 6 months before you can have breast augmentation surgery. Because augmentation surgery to be performed when the milk channels are full increases the possibility of infection of the silicone and makes the decision of the final shape difficult.

You will be discharged from the hospital on the next day after your tests are done 1 day before the surgery, your surgery will be performed the next day, and you will rest in the hospital in the evening. As a result, it is enough to stay 2 nights in Ankara for breast augmentation surgery. However, depending on the procedure, you should come back to Ankara for your first control after 2 or 4 weeks. There is no harm in traveling after the surgery.

Breast augmentation surgery is a very common type of surgery with minimal risk. Make sure you are ready before the operation, have done enough research, and decide to have this surgery for the right reasons.
