breast augmentation It is an operation to enlarge the existing breast volume by injecting one’s own fat or by means of a breast prosthesis. Breast augmentation is an operation to enlarge the existing breast volume by injecting one’s own fat or by means of a breast prosthesis.
WHAT IS THE IMPORTANCE OF BREAST FOR WOMEN?
Breasts, besides being the symbol of motherhood, nutrition and reproduction, are organs that show the woman’s femininity and charm. It is the secondary organs that provide milk and life-giving properties, especially for women, are important in sexual intercourse and have sexual sensitivity. In cases where the breast tissue is not developed or the only one is removed, the body integrity and gender perception of the woman is damaged. Although the absence of the breast is camouflaged with special bras, the absence of these feminine symbols can cause demoralization and even depression in the patient. This situation can keep people under pressure both psychologically and socially, people’s self-confidence decreases and it can cause negativities in bilateral relations.
WHO IS MOST COMMON BREAST ENLARGEMENT SURGERY?
Mostly, breasts remain small due to insufficient breast tissue developmentally, or insufficient growth, and there is no reason for this to a large extent. In addition, since the breasts undergo involution after pregnancy, they may shrink even if they are normal before pregnancy.
WHAT ARE BREAST ENLARGEMENT SURGERIES?
Two methods are used as surgery. The first is the method in which we take the patient’s own fat and inject it into the breasts, the second breast prosthesisis
IS FAT INJECTION FOR BREAST ENLARGEMENT APPLICABLE TO EVERY PATIENT?
breast augmentation Purposeful fat injection is a method that can be applied in a limited patient group. In particular, it can be applied to increase the fullness in the degague part and to eliminate small-scale asymmetries and shape deformities. Of course, having one’s own fat is a good thing, but disadvantages such as the risk of infection, the ability to provide low volume, the need for a second session, and the difficulty of the diagnostic approach in terms of breast diseases are the main reasons for the limited application.
IS THERE A NON-SURGERY METHOD FOR BREAST ENLARGEMENT?
The only healthy method that can be applied outside of surgery for breast augmentation is to use a supported bra. Medicines, creams, organic tablets, etc. for breast enlargement should not be trusted even if it is said that the products are natural. There must be a hormonal effect in the product that can enlarge the breast tissue. As a result of this hormonal effect, you may experience serious health problems and increase your risk of cancer. Never use breast enlargement products sold under the name of herbal products, food supplements, etc.
WHAT IS USED FOR BREAST ENLARGEMENT?
Silicone breast prosthesisused.
WHAT IS THE STRUCTURE OF SILICONE?
The basic building block is silicon. It is the general name given to the members of the polymer family obtained by adding various groups to the silicon-oxygen chain. Sea sand, crystal, quartz is basically silica and is a very common material. Silicon is formed by heating and processing silica with carbon at a high temperature. This substance, which becomes liquid, gel or rubbery by further processing, is called silicone. Silicones are found in many products that we frequently use in our daily lives, such as sun and hand lotions, soap, processed food, chewing gum, and polish.
DOES BREAST PROSTHESIS EXPLORE?
Solid gel silicones used today are very, very difficult to explode. It is so difficult that even if a car passes over it, we can say that it will explode. You can slice these prostheses like bread. They are damaged by a sharp tool rather than an explosion. In addition, in case of explosion, silicone does not flow like water, does not lose its shape, only creates a bird’s eye shaped protrusion.
ARE BREAST PROSTHESIS ONE TYPE?
Breast prostheses It has diversified over time to cater to different needs. There are many varieties.
The surface may be rough or smooth. All prostheses have 1-2 mm thick silicone on the outer wall, inside this outer silicone wall, there may be gel silicone or salt water. It can be round in shape or in the form of anatomical drops. It can be between 100-800 cc in volume.
DOES A BREAST PROSTHESIS SURGERY STOP THE BODY?
Prosthesis; It can be placed on the breast in three ways: nipple, submammary groove and armpit. An indistinct trace remains in all three regions.
WHAT IS THE DIFFERENCE OF THE REGIONS WHERE THE PROSTHESIS IS PLACED?
If the nipple is of a certain size, the prosthesis is placed by making a half-moon incision at the junction of the colored part and the normal skin. The vague-indistinct trace remains at least with this method. Affecting the nipple sensation and damaging the milk ducts has a higher risk ratio than other regions.
In incisions made under the armpit, there is no scar on the anterior chest wall, there is a scar under the armpit. With the incisions made in this area, you have to use submuscular prostheses filled with salt water, you cannot use anatomical or round shaped gel-filled prostheses. Working with full control from the armpit to the midline of the chest is not possible in every patient, even endoscopically, and the result can sometimes be open to surprises.
The prosthesis is placed through an incision of 3-4 cm, from the submammary groove to the groove. There is no damage to the breast sense and milk ducts.
DOES BREAST PROSTHESIS OBSTACLE BREASTFEEDING?
Prosthetic surgery is not an obstacle to breastfeeding. In addition, current studies have shown that the milk of women with prostheses does not have higher levels of silicon than the milk of women without prostheses. Studies have shown that there is more silicon in cow’s milk and baby food than in the milk of women with breast prostheses.
IS THERE A CONDITION TO CHANGE BREAST PROSTHESIS AFTER A SPECIAL TIME?
In the past, prostheses containing salt water, which were used frequently, deformations occurred over time (approximately 5-10 years) and had to be changed. However, we do not encounter such a situation in solid gel-filled silicone prostheses used today. The only exception is the development of capsular contracture.
CAN A PROBLEM OCCUR AFTER BREAST PROSTHESIS SURGERY?
In the early period, infection, bleeding, opening of the sutures, and blood collection, which we call hematoma, may occur in any surgery. These conditions are very rare, but you should know before the operation, even if it is rare. Capsular contracture may be seen in the late period.
WHAT IS CAPSULE CONTRACT?
The body perceives the implanted prosthesis as a foreign body and creates a physiological barrier around it to keep it in a limited area in the body. This barrier is exaggerated in 0.5-1% of patients and turns into a structure called a capsule. In advanced capsules, the shape of the breast is distorted.
WHAT IS THE TREATMENT OF CAPSULE CONTRACT?
There is no need to intervene in capsule reactions below a certain level. However, in patients with advanced breast deformities, the prosthesis should be replaced simultaneously with cutting or removing the capsule as a treatment. If the prosthesis is placed under the breast in the first surgery, it can be placed under the muscle in the second surgery. In other words, different treatment alternatives come to the fore according to the patient.
WHAT SHOULD BE CONSIDERED TO AVOID CAPSULE CONTRACT?
We do not know in which patient and when the capsule contracture will occur – we do not know. The rough surface of the prosthesis, their placement under the muscle, the absence of infection and bleeding after the surgery, and keeping foreign objects, including glove powder, away from the surgical field significantly reduce the risk.
WHERE IS THE PROSTHESIS PLACED IN BREAST ENLARGEMENT SURGERY?
Prostheses can be placed under the muscle or on the muscle, depending on the patient’s chest wall structure and the amount of breast tissue, and under or over the muscle membrane for supramuscular placement. Placed regions have their own pros and cons. When the prosthesis is placed under the muscle, it is protected by the muscle tissue and the risk of capsule formation is reduced. When placed on the muscle, it looks more anatomical and natural. However, in order to place it on the muscle, a certain amount of breast tissue is required.
IS IT ENOUGH TO PUT PROSTHESIS ON drooping and small breasts?
If the sagging of the breasts, which we call ptosis, is low and the breasts are smaller than the ideal size, it can only be recovered by placing a prosthesis. However, if the sagging is advanced and the breast volume is low, a prosthesis should be placed in combination with breast lift surgery, which we call mastopexy.
I’M TIRED OF THE PROSTHESIS, I WANT TO REMOVE IT
You are tired of your dentures and now you want to have them removed. Your prostheses can be removed even with local anesthesia. If it is not high volume, your breasts will not change much after your prostheses are removed. You can continue your life from where you left off without prosthesis.
DO PROSTHESIS MAKE BREAST CANCER DIAGNOSIS DIFFICULT?
It is an incorrect statement to say that prostheses delay the diagnosis of breast cancer and reduce the chance of early diagnosis. They do not pose any difficulties in advanced imaging methods such as MRI, USG, CT used today. Only the presence of prostheses should be told to the radiologist.
HOW IS THE PROCESS OF BREAST ENLARGEMENT SURGERY IN BRIEF?
You will first be examined by your doctor. Then, together with your doctor, you decide on the size, shape and place of placement of the prosthesis to be placed jointly. The surgery is performed under general anesthesia and takes an average of 1.5 hours. After the surgery is finished, you will be put on a shaping bra and taken to your room. You will be accommodated in the hospital for a maximum of one day. We prefer you to lie on your back and hips with an inclination of 30-45 degrees, especially for the first 3 days after the surgery. You use oral painkillers and antibiotics for five days. You can take a bath on the 3rd day after the operation, and your dressing will be done on the same day. You have no stitches to be removed. The bra should be used for 3-6 weeks.
WHAT SHOULD BE ATTENTION TO AFTER BREAST ENLARGEMENT SURGERY?
Especially in the first week, excessive arm movements should be avoided because the prosthesis can be displaced. There is no unbearable pain, if it does, it can be controlled with simple painkillers. You may feel tightness in your breasts in the early period, it will decrease over time. After about 3 months, it will appear 10-20% smaller than its size in the early period with the disappearance of edema.
